Discharge Planning

Planning from Day One

Discharge planning begins on the day of admission. From the outset, your treatment team will be working towards a clear set of goals that will determine when you are ready to leave and what support you will need afterwards.

The Discharge Plan

Before you leave, your consultant will develop a comprehensive discharge plan including: a summary of your treatment and progress; ongoing medication (if applicable); follow-up outpatient appointments; relapse prevention strategies; and communication with your GP.

Aftercare

We offer ongoing outpatient follow-up for all patients who wish to continue their care with us. For residential patients, we recommend a period of step-down to day care or outpatient treatment to consolidate the gains made during your stay.

GP Communication

With your consent, we will provide your GP with a detailed discharge summary including diagnosis, treatment received, current medication, and recommendations for ongoing care.
